Your Office Visit

Our cardiac care begins with an interview and an examination. At which time we fully explain your condition and treatment plan. We encourage you to ask questions and possibly have a family member present at each visit.

  • Office Hours: By Appointment Only Monday thru Friday 8:00 AM to 5:00 PM

  • We request that all initial evaluations and testing be scheduled through your primary care physician.

  • For Your Office Visit:
    • Please arrive 10 minutes prior to your scheduled appointment time to allow time to complete the necessary paperwork.
    • If your appointment is at our office at Genesys Regional Medical Center, please allow extra time for parking, and remember that the hospital supplies a courtesy shuttle for your convenience.
    • Bring all of your insurance cards with you.
    • Bring a list of your current medications including strength as well as how many times a day the medication is taken.
    • Bring copies of any pertinent medical records from other medical providers, including test results and lab work.
    • We would appreciate the completion of the patient information form and health assessment form prior to your visit. Please bring them with you.
  • PLEASE NOTE: You must have Adobe Acrobat installed in order to access the PDF files above. Please take a moment to download this free software if you do not already have it.

  • We do our best to avoid lengthy waiting periods when you are in our office for your appointment. However, our doctors must respond to emergencies as a priority and we ask for your patience.

  • Prescription Refills: Please call during regular office hours to renew prescriptions. It is best
    to call 2-3 days before your medication runs out. For your convenience, voice mail is now
    available for prescription renewal. At the sound of the beep please leave the following information:
    - after each submission enter the # key

    • your name, please spell your last name,
    • your date of birth,
    • your telephone number,
    • pharmacy name and pharmacy telephone number,
    • medication name, medication strength and the number of times you take the medication daily,
    • the name of the Doctor you see in our office.

Your refill will be called in within 24 hours.

  • Occasionally emergencies arise and it will be necessary to change your appointment, if this
    occurs we will contact you as soon as possible.

  • If you are unable to keep your appointment, please try to notify us at least 24 hours in advance.

  • In case of emergency, our doctors are available 24 hours a day. If you have an urgent problem or question, please call our office and talk to our staff. If your condition requires an appointment, one will be scheduled promptly. Always remember in a life threatening situation, dial “911” to be taken to the nearest emergency room.

Fees and Insurance

  • We are committed to providing quality care at the lowest cost possible. We participate with most insurance companies and will file your claim for you.
  • We request payment at the time service is provided, and encourage you to discuss fees with us prior to your appointment. Payment arrangements may be made in advance with the billing department. If you have any questions, please call our billing department at (810) 579-1701.
  • If you are an HMO patient, it is your responsibility to obtain a referral from your primary care physician for every visit to our office.
